We are the single point of access (SPA) for the MK Integrated Falls Prevention Service. All fall referrals should be directed to the B-Well Group in Milton Keynes
We offer a dedicated online MK Falls database that can be accessed remotely by health and social care professionals in Milton Keynes who work with older adults.
We provide clinical triage for all referrals received by the SPA, conducted by senior clinicians.
We provide well-being and multi-factorial assessments to identify fall risks and develop individualized, tailored fall prevention management plans.
We run a variety of evidence-based exercise classes to help older adults stay as active and independent as possible. All classes are designed according to each client's ability and focus on fall prevention.

Usually 50+ years old Registered with MK GP's or Living in MK Council Boundary History of fall identified at risk of fall or fear of falling
